Turn a vague disability handoff into a claim record another worker can pick up
Disability application submitted.
That is the whole handoff.
The record has one screenshot. The client remembers answering questions online. There is no program name, no confirmation number, and no way to tell whether Social Security has a signed claim or an unfinished application waiting behind a reentry number.
The green checkmark may prove that a screen worked. The case still needs a name.

My field rule is simple: no program name, no usable status.
Start with the paper or screen in front of you. Look for Supplemental Security Income, Social Security Disability Insurance, Title XVI, Title II, or both. If no document names the program, call Social Security and ask staff to check for an open Title II disability claim and an open Title XVI Supplemental Security Income claim.
One folder label can hide two claims. Split them before the next worker inherits the fog.
Let the case tell you which lane is open
The client reports twelve years of steady work, $4,000 in savings, and no income now. A quick screen may close Supplemental Security Income because the savings exceed the ordinary individual resource limit. The work-based claim is still sitting there.
Social Security Disability Insurance, or SSDI, follows the person's covered work record. Social Security checks the earnings record, age, and disability onset date to decide whether insured status exists. Five of the last ten years is a common adult screen. Younger workers can qualify with fewer credits. A resume can rebuild employer names and dates. The Social Security earnings record decides insured status.
Savings, a home, and ordinary unearned income do not control the SSDI work-credit lane.
Supplemental Security Income, or SSI, follows income, countable resources, living arrangement, residency, and citizenship or eligible-noncitizen rules. In 2026, the maximum federal payment standard is $994 per month for an eligible individual and $1,491 for an eligible couple. The countable-resource limit is $2,000 for an individual and $3,000 for a couple. The home a person lives in, one vehicle used for transportation, household goods, and some other resources may be excluded.
Use those numbers for screening. Do not promise a payment from them. SSI counts income and living arrangements under separate rules, and the final amount belongs to the agency record.
Now change the facts. The client has a short or uneven work record and very little income or savings. SSI may be the stronger lane. A low expected SSDI payment can also leave SSI open. Social Security calls the two claims together a concurrent claim.
The medical standard overlaps. For an adult disability claim, Social Security asks whether a medically determinable condition prevents substantial gainful activity and is expected to result in death or last at least 12 continuous months. The nonmedical questions split after that.
Leave the first conversation with one line for each lane:
- SSI: appears open, appears closed, or needs Social Security confirmation.
- SSDI: appears open, appears closed, or needs an earnings-record and insured-status check.
- Concurrent: both claims are named, or the second lane still needs to be checked.

The notice title beats a remembered interview, a payment guess, or the diagnosis written on the intake form.

Find out what submitted means
Once the program is named, inspect the screenshot.
An online start, an appointment request, a protected filing date, and a signed application are separate records. Case notes flatten all four into applied. That shortcut makes the next call longer and the filing date harder to protect.
Track four records:
- First contact: date, time, route, programs named, safe claimant contact, and worker role.
- Protected date and closeout: program protected, notice date, exact closeout date, reentry number, and appointment confirmation.
- Valid signed application: program submitted, signature status, submission date, route, and application summary.
- Confirmation and next action: receipt or confirmation number, current office or component, missing item, due date, owner, and next check date.

Some phone contacts, appointment requests, online claim starts, or signed written statements can establish a protected filing date. The protected date becomes the application date when a valid application is completed within the controlling closeout period. A Title II closeout generally runs six months. A Title XVI closeout generally runs 60 days. The notice or appointment confirmation gives the date to use.
The calendar on the notice outranks calendar math from memory. Read it. Save it. Enter the date exactly.
The client can make first contact before every medical record is gathered. Social Security tells applicants not to delay filing while documents are missing. Start the supported route, then build the evidence record.
When the note says only started online, look for the reentry number, appointment confirmation, application summary, electronic signature status, and receipt. An empty proof field keeps the filing question open.
Use this call script:
Please check whether there is an SSDI claim, an SSI claim, or both. We need the first contact date, any protected date and closeout date, signed-application status, current component, and next action.
A worker can help complete the application. The claimant or another proper applicant ordinarily signs it. General help with the form does not create formal authority over the claim. Ongoing representation uses Social Security's appointment process, commonly Form SSA-1696.
Worker move: Before closing the contact, save one item for each open record: first contact, closeout, signed application, and confirmation.

Run the ten-minute screen without closing a lane
Record current work and gross earnings, self-employment, the date work stopped or changed, when the condition began affecting work, expected duration, and whether Social Security's blindness rules may apply.
The 2026 substantial gainful activity screen is $1,690 per month for a person who is not blind and $2,830 per month for a person Social Security considers blind. Self-employment and blindness have additional rules. These figures screen the case. Social Security makes the work and medical decisions.
For SSI, ask about current income, possible countable resources, living arrangement, residency, and any citizenship or eligible-noncitizen review.
For SSDI, ask whether anyone has checked the current Social Security earnings record. When the client's work history and the earnings record disagree, document the year, employer, amount shown, amount expected, and available wage or tax proof. Keep the SSI screen moving while Social Security reviews the earnings issue.
Two entries belong in the correction pile:
Client worked, so SSDI is open.Work history starts the question. The agency record answers it.Client has savings, so disability is closed.Savings may affect SSI. They do not close the SSDI work-credit lane.
Medicare generally begins after a 24-month qualifying period tied to SSDI entitlement. Record the entitlement date and keep current health coverage active until the Medicare effective date is confirmed.
Worker move: Write one nonmedical screen for SSI and a separate insured-status screen for SSDI. If either answer depends on an agency record, label it pending instead of guessing.

Use a door the claimant can actually reach
Across Hampden, Hampshire, Franklin, and Berkshire counties, the first claim door is Social Security.
Start at ssa.gov/apply when the online flow fits. Otherwise call 1-800-772-1213. The TTY number is 1-800-325-0778. Before planning an office visit, enter the claimant's address or ZIP code in the Social Security office locator. Social Security currently says to make an appointment before visiting a local office.
The Massachusetts regional page lists field-office cities in Holyoke, Springfield, North Adams, and Pittsfield. It lists no field-office city in Hampshire or Franklin County. The city list does not assign a claimant's office. The locator controls the current service area, address, hours, closures, and office details.
If the online identity check fails, save the error screen and reentry number. After two failed identity checks, some claimants receive an option to schedule a telephone or in-office appointment through the online flow. The option does not appear in every case. The appointment confirmation can carry protected-filing closeout dates. The signed application remains a separate record.
If the option never appears, call Social Security and name both programs being screened.
Social Security provides free interpreter service by telephone and in an office. Spanish callers can press 7. Callers using another language should remain silent through the English automated prompts until a representative answers. Record the language or accommodation requested, request date, appointment route, confirmation, and response.
When the claimant lacks a fixed, regular, and adequate nighttime residence, or expects to lose the current place within 14 days without another adequate place, ask whether the homelessness flag is recorded. Keep the safe mailing route, phone or email, authorized third-party contact when one exists, and the agency response. Priority handling changes processing. It carries no approval promise.
An access barrier should leave an access record: error screen, call log, appointment confirmation, interpreter request, or accommodation response. Client did not follow through turns a system failure into a character judgment and gives the next worker nothing to use.
Worker move: Match every failed door to proof and an alternate route. Save the error. Make the call. Write the appointment date and closeout date in the same note.

Track the office that owns the next move
Social Security opens the federal claim, checks nonmedical requirements, establishes the filing record, sends the medical claim to the state when appropriate, and issues federal notices.
For a quiet claim, use the application-status page or call 1-800-772-1213 and say application status. Save the status date, program, current component, last completed action, next action, and next check date.
MassAbility Disability Determination Services, or DDS, develops the medical claim after Social Security sends it to the state. DDS does not open the first SSI or SSDI application. Once the introductory letter arrives, use the Vocational Disability Examiner named there.

Ask the examiner:
What form, record, examination, or claimant action is still open? What date is it due, and where should the response go?
If a consultative-examination notice arrives late, the claimant cannot attend, or the appointment was missed, call the DDS number on the notice immediately. State the reason. Ask about rescheduling, interpreter support, and travel reimbursement when they apply. Keep the full notice and envelope, date received, call record, answer, and any new appointment date.
A quiet claim still has an owner. Find that owner before opening another application and giving the same case two front doors.
Worker move: Every status note needs the program, current component, last completed action, open action, owner, due date, and next check date.

Keep the federal claim separate from the benefits around it
An SSI approval can trigger other Massachusetts work. Keep those records beside the federal claim, not inside it.
Massachusetts residents approved for SSI automatically enter MassHealth under current state rules. Verify that the eligibility record arrived, the coverage type and effective date are correct, and the member address is current. MassHealth Member Customer Service is 1-800-841-2900. TDD or TTY is 711. The Springfield Enrollment Center at 243 Cottage Street is the only Western Massachusetts center on the current state list. Walk-in service is limited, and MassHealth encourages appointments.
The Massachusetts State Supplement Program is a separate state payment. A person must first apply for SSI. Someone above the federal SSI income limit may still have a state lane under current rules. The statewide number is 1-877-863-1128. Keep the state notice, living-arrangement record, payment decision, effective date, and requested verification in its own record.
Community Legal Aid handles some Social Security and public-benefit matters for people in Central and Western Massachusetts. Central intake is 855-252-5342. Acceptance depends on legal priorities, financial eligibility, the case facts, and capacity. Save the complete denial notice and envelope before making the connection.
Food, housing, medication, transportation, and cash needs keep moving while the federal claim is pending. Track those bridges without letting them replace the open Social Security action.
Worker move: Keep four owner lines when they apply: Social Security claim, DDS medical development, MassHealth coverage, and State Supplement payment.

Rewrite the handoff before you close
The old note said:
Disability application submitted.
The new note should read more like this:
SSI and SSDI both screened. SSDI signed application submitted online July 21; confirmation saved. SSI appointment requested July 21; protected date and September 19 closeout shown on confirmation. Social Security owns the SSI application interview. Worker will check status July 28.
Use the real facts from the case. Keep Social Security numbers, portal credentials, bank details, and detailed diagnosis narratives out of a shared handoff.
If a notice closes one lane, read the program title and reason. An SSI nonmedical closure leaves the SSDI question open until its own record answers it. Medical development shows that a disability claim reached DDS. It does not prove the SSI nonmedical screen passed. Save the full notice and envelope before deciding whether the next move is correction, appeal, or a new application.
